Why slow breathing has a physiological effect
Slow, controlled breathing acts on the autonomic nervous system through a measurable mechanism. Heart rate naturally rises slightly on inhalation and falls on exhalation — respiratory sinus arrhythmia — mediated by the vagus nerve. Extending the exhalation lengthens the parasympathetic phase, which is why exhale-weighted patterns have a calming effect that inhale-weighted ones do not.
Breathing at roughly six breaths per minute, about ten seconds per cycle, produces a resonance effect where heart rate variability and blood pressure oscillations synchronise. This resonance frequency varies between individuals but clusters around that rate, and it is the basis of heart rate variability biofeedback.
The evidence base is real but modest in effect size. Controlled trials of slow breathing show reductions in self-reported anxiety and small reductions in blood pressure, and a widely cited 2023 randomised study found cyclic sighing — a double inhale followed by an extended exhale — outperformed both mindfulness meditation and other breathing patterns for daily mood improvement over a month. These are useful effects, not dramatic ones.
The common patterns
Box breathing uses equal counts for inhale, hold, exhale and hold, typically four seconds each. It comes from military and emergency-services training, and its virtue is that the symmetry is easy to remember under stress.
The 4-7-8 pattern — inhale four, hold seven, exhale eight — is strongly exhale-weighted and is often suggested for sleep onset. The long hold is demanding for beginners, and shortening all three proportionally while keeping the ratio works just as well.
Coherent or resonance breathing simply uses equal inhale and exhale of five to six seconds with no holds, giving about five to six breaths per minute. It is the gentlest of the three and the one best supported for sustained practice, since it can be maintained for ten or twenty minutes comfortably. Physiological sighing — two inhales through the nose then a long exhale through the mouth — is the fastest-acting for acute stress and works within a few breaths.
Practising safely
Breath holds and rapid breathing carry real risks that gentle patterns do not. Hyperventilation lowers carbon dioxide and can cause dizziness, tingling in the hands and face, and fainting. Never practise breath-holding techniques in or near water, or while driving — shallow water blackout from hyperventilation before breath-hold diving has caused deaths, including among experienced swimmers.
Some people should be cautious or should seek advice first: anyone with a respiratory condition such as asthma or COPD, cardiovascular disease, epilepsy, a history of fainting, or who is pregnant. Intense techniques involving prolonged holds or forceful breathing are not appropriate without guidance.
Stop if you feel light-headed, and return to normal breathing — the goal is a comfortable extension of the natural rhythm, not a strain.
